To subscribe to AutoSweep RFID, just follow these simple steps:
1. Download and fill up the following forms:
NOTE: If you have more than 5 vehicles to be registered under Personal Account, please fill up the Vehicle Assignment form and attach it along with the subscription form.
2. Attach Your Supporting Documents
- Photocopy of the OR & CR of the vehicle you want to enroll.
- If you are not the registered owner, please secure an authorization letter from the registered owner. Please attach in the authorization letter the photocopy of his/her ID with original signature on the side.
- Photocopy of any valid government issued ID (preferably driver’s license).
3. If you wish to enroll your account to automatic replenishment via credit card, please download and fill up the Auto Debit Arrangement form and attach the photocopy of your credit card (front only).
NOTE: If the credit card you will enroll is not under your name, please fill up and attach the Authority to Use Credit Card form along with your ADA form.
4. Submit all documents to any of our installation sites. INCOMPLETE DOCUMENTS WILL NOT BE PROCESSED.
Please make sure you bring the actual vehicle during the installation. If you are sending a representative to process you subscription, please provide an authorization letter along with his/her valid ID.
